重要提示：閣下(申請者)須仔細閱讀以下事項。

1. 申請人必須在收到以他/她的名義所發出的會員卡後立即在卡上簽署。
   Please sign up immediately after the applicant receives own membership card.
2. 申請人成為會員後，應小心保管其會員卡並確保其會員卡由其本人持有及使用，若有遺失，應即時通知本中心。
   For members, the membership card must be kept carefully and guaranteed used by the cardholder only. Any loss of the membership card must be informed to Whole Person Development Institute (WPDI) immediately.
3. 會員卡如有遺失或損毀，補領手續為每張港幣50元。
   Any loss or destroy of the membership card, $50 compensative procedures fee shall be given.
4. 會員卡只供本中心會員使用，不得借用或轉讓。如有任何疑問，將以卡上簽名為準。
   The card is only applicable to principle cardholder and it is not allowed to borrow or transfer. In case of dispute, signature on the card shall be final embraced.
5. 申請個人會籍及家庭會籍主卡之人士必須年滿18歲。
   Applicant who apply the individual membership or the core member of family membership shall be 18 years old or above.
6. 申請人成為會員後，若作出欺騙行為、嚴重疏忽或未能在發現其會員卡已遺失後合理可行地盡早通知本中心，則會員將須承擔與會員卡有關損失。
   For members who make the fraud, the gross negligence or not inform WPDI after discovering the loss of membership card immediately without reasonable explanation, the member will probably undertake with the membership card related loss.
7. 申請人資料絕對保密，只供中心內部紀錄及中心推廣宣傳之用。
   The personal information will be kept at confidential and only use for internal record and promotion purposes.
8. 若會員未能支付到期需付之年費，本中心可能終止會員之會籍並不作另行通知。
   For any memberships’ payment default, WPDI reserves the right to terminate the membership of the applicant without notice.
9. 本中心可核實申請人所提供之任何資料，並在其認為合適的情況下向該等資料來源取得進一步資料。
   WPDI reserves the right to verify the information of the applicant if necessary and ask for further information under the agreement of the applicant.
10. 請將表格寄回香港北角英皇道338號華懋交易廣場2期16樓1601-1603室(會員服務部收)或傳真至2881 0163。
    Please post the application form to Unit 1601-1603, 16/F, Two Chinachem Exchange Square, 338 King’s Road, North Point, Hong Kong.
11. 申請人若以劃線支票付款，抬頭請寫上『全人發展中心』或『Whole Person Development Institute』。
    For the cheque payment, a crossed cheque shall be made payable to 『Whole Person Development Institute』
12. 如有任何爭議，本中心保留最終決定權。
    In case of dispute, decision of WPDI shall be final.
13. 如條款細則/聲明之中文版及英文譯本有任何差異，一切以中文本為準。
    In case of discrepancy between the Chinese version of terms and conditions / declaration and its English translation, the Chinese versions shall prevail.

本人證實以上資料均屬詳實，並清楚明白以上之條款及條件之使用，並同意於本申請批核後予以遵守及受該等條款及條件所約束。
